Evidence Checklist
Smart Contract Vulnerability Cover
Proof of Loss and Ownership for Smart Contract Vulnerability Cover shall include, but is not limited to:
a transaction of one (1) USD stablecoin from the Claimant’s Covered Address to Cover Purchase Address and back to the Covered Address within 7 days after Claim submission;
a snapshot of the impacted Covered Address’ balances at block times directly before, during and after any losses have been applied;
a report from a Professional Security Firm describing the Claimable Risk Event;
references to any relevant on-chain transactions; and/or
any other evidence as deemed necessary by the Protocol.
Stablecoin De-Peg Risk Cover
Proof of Loss and Ownership for Stablecoin De-Peg Risk Cover shall include, but is not limited to:
a transaction of one (1) USD stablecoin from the Claimant’s Covered Address to Cover Purchase Address and back to the Covered Address within 7 days after Claim submission;
a snapshot of the impacted Covered Address’ Stablecoin Balance at block times directly before, during and after any losses have been applied;
a snapshot of the impacted Covered Address’ Stablecoin Balance 10 days before the Claimable Risk Event is triggered;
a report from a Professional Security Firm or the Stablecoin issuer(s) describing the Claimable Risk Event;
references to any relevant on-chain transactions; and/or
any other evidence as deemed necessary by the Protocol.
Custodian Risk Cover
Proof of Loss and Ownership for Custodian Risk Cover shall include, but is not limited to:
screenshots and a single video recording of withdrawal suspension notices / losses recorded on account statements under the Covered Account as identified by the Covered User ID;
official social media statements and emails from the Designated Custodian with regards to the Claimable Risk Event;
an official report or description of the Claimable Risk Event from a Professional Security Firm or the Designated Custodian;
any supporting materials to determine the value of the Cover Purchaser’s lost assets;
references to any relevant on-chain transactions; and
any other evidence as deemed necessary by the Protocol.
Post-Audit Cover
Proof of Loss and Ownership for Smart Contract Vulnerability Cover shall include, but is not limited to:
a snapshot of the impacted Designated Smart Contract balances at block times directly before, during and after any losses have been applied;
a report from a Professional Security Firm or the Auditor describing the Claimable Risk Event;
references to any relevant on-chain transactions; and/or
any other evidence as deemed necessary by the Company.
Ethereum Slashing Cover
Proof of Loss shall include, but is not limited to:
CSV file of a list of slashed Covered Validator(s)’ public keys;
a statement, notice or report from the Staking Platform identifying the Claimant’s slashed Covered Validator(s) as operating under the Staking Platform during the Claimable Risk Event;
a statement, notice or report from the Staking Platform identifying the cause of the Claimable Risk Event;
references to any relevant on-chain transactions; and/or
any other evidence as deemed necessary by the Protocol.
